The value of CHA2DS2-VASc-HSF score for prediction of severity of coronary artery disease in patients undergoing elective coronary angiography assessed by GENSINI score.
Background: Association of CHA2DS2-VASc-HSF score with prediction of coronary artery disease in stable CAD patients. Aim: Association of CHA2DS2-VASc-HSF score with prediction of coronary artery disease assessed by Gensini score. Subjects and methods: Prospective cohort study performed over a period of 6 months from July 2022 to January 2023 at department of Cardiology, Benha Health Insurance Hospital, and included 100 patients who underwent elective invasive coronary angiography due to suspected CAD. CHA2DS2-VASc-HSF score and Gensini score were calculated for all patients. All subjects underwent full history taking, clinical examination, Electrocardiogram and Echocardiography. Results: We included 100 patients in the study (mean age 60 ±11 years, male predominance 65%, diabetes 56%, hypertension 65%, dyslipidemia 54%, smoking 52%, history of CAD 7%, previous stroke 11%, family history of CAD 32%, peripheral arterial disease 6%). All variables were included in analysis. One year increase in age associated with a 5% increased risk of high-degree-stenosis. Diabetes associated with seven times increased risk of high-degree-stenosis. Hypertension associated with eight times increased risk of high-degree-stenosis. Dyslipidemia associated with eight times increased risk of high-degree-stenosis. Two to three vessels affection associated with nine times increased risk of high-grade-stenosis. One unit increase in the CHA2DS2-VASc-HSF associated with six times increased risk of high-grade-stenosis. A stepwise analysis done for these variables. Variables that remained in the model were number of vessels affected and CHA2DS2-VASc-HSF. Conclusion: High CHA2DS2-VASc-HSF score showed increased number of diseased vessels and Gensini score.
